    SPORTS PHYSICALS WITH THE SCHOOL DOCTOR:

    The next date for sports physicals with the school doctor is:

    Tuesday, November 12th 2024 at 10:00 AM.

    ***CLICK HERE for Online Permission and Participation forms - must be completed before you can have a physical with the school doctor.

    All student athletes having a school physical must arrive at the health office with their completed PPE forms.  

    • All Student Athletes: visit the STUDENT ATHLETE INFORMATION tab for important updated information and instructions to complete the recently updated PPE forms.

    • All New Students: visit the NEW STUDENT INFORMATION tab to view the welcome letter.

    • All Students/Parents: visit the HEALTH OFFICE FORMS tab for all required forms for physicals, sports physicals, allergies, asthma, or medications at school.

    ILLNESS: If your child is sick, PLEASE keep him/her home. Students MUST be fever-free for 24 hours without fever-reducing medication before returning to school (fever is defined as subjective fever (feeling feverish) or a measured temperature of 100.4°F or higher). Call the attendance office at ext. 3010 to report the absence. If you take your child to the doctor, request a note and send it in with your child upon their return to school.

    GYM EXCUSES: If your child cannot participate in the full gym program due to an injury or other medical condition, a note from a parent/guardian is required. Any disability lasting longer than 2 days requires a note from a medical physician, and must include any specific restrictions and/or additional school accommodations.
