All student athletes must complete the online Permission and Participation forms to participate in school sports. Permission and Participation forms must be completed online each school year to participate in school sports.
Starting July 1st 2024, the state-mandated PPE forms have been updated and MUST be used for any student having their physical on or after July 1st 2024. The previous PPE forms packets cannot be accepted after July 1st 2024.
Students having physicals with the school doctor: report to the school nurse for height, weight, blood pressure, and vision screenings a minimum of 2 days before the scheduled date for physicals. The school doctor will not complete your physical exam without this data.
Students having physicals with their family doctor: submit completed PPE Medical Eligibility forms to the school nurse a minimum of 2 days before the scheduled date for school physicals (this is when the school doctor will sign off on your physical for final sports clearance).

Scholastic Student Athlete Safety Act - Information for Parents/Guardians
Prior to participation on a school-sponsored interscholastic or intramural athletic team or squad, each student-athlete in grades six through 12 must present a completed Preparticipation Physical Evaluation (PPE) Medical Eligibility form to the designated school staff member. Important information regarding the PPE forms is provided below. Please read through these instructions completely, as the information has been updated as of July 1st, 2024.

The PPE may ONLY be completed by a licensed physician, advanced practice nurse (APN) or physician assistant (PA) that has completed the Student-Athlete Cardiac Assessment Professional Development module. It is recommended that you verify that your medical provider has completed this module before scheduling an appointment for a PPE in order to not delay clearance for your child to play sports.

The required PPE must be conducted within 365 days prior to the first official practice in an athletic season. The PPE Medical Eligibility Form is available along with additional required forms on the school's website.

The parent/guardian must complete the History Form. This form should be maintained by the healthcare provider completing the physical exam (medical home). It should not be shared with schools.

The parent/guardian must complete the supplemental Athlete With Disabilities Form (if applicable). This form should be maintained by the healthcare provider completing the physical exam (medical home). It should not be shared with schools.

The licensed physician, APN or PA who performs the physical examination must complete the Physical Examination Form and the PPE Medical Eligibility Form. The Physical Examination Form should be maintained by the healthcare provider completing the physical exam (medical home). It should not be shared with schools. The PPE Medical Eligibility Form is the only documentation required to be submitted to the school.

The licensed physician, APN or PA must also sign the certification statement on the PPE Medical Eligibility Form attesting to the completion of the professional development module. Each board of education and charter school or nonpublic school governing authority must retain the original signed certification on the PPE Medical Eligibility Form to attest to the qualification of the licensed physician, APN or PA to perform the PPE.

The school district must provide written notification to the parent/guardian, signed by the school physician, indicating approval of the student’s participation in a school-sponsored interscholastic or intramural athletic team or squad based upon review of the medical report, or must provide the reason(s) for the disapproval of the student’s participation.

For student-athletes that had a medical examination completed more than 90 days prior to the first official practice in an athletic season, the Health History Update Questionnaire (repeater) form must be completed, and signed by the student’s parent/guardian. The repeater must be reviewed by the school nurse and, if applicable, the school’s doctor or athletic trainer. Click here to be directed to the Health History Update Questionnaire.
