School Doctor Physicals

Next school physicals will be in May/June for soccer.

You must complete the online permissions for school physicians to complete your physical.  All permissions for sports physicals are due at least 1 day prior to physicals in order to have a spot reserved for you. Sports permissions can be found on the school ATHLETICS tab. First-come, first-served basis. THIS WILL BE THE ONLY PHYSICAL BEFORE SOFTBALL AND BASEBALL. Any questions email the school nurse at kellyn@mcmsnj.net.  

 Any student wishing to participate in sports or is a new student in 9th or higher grade is required to have a current physical and may use the school doctor to obtain the needed physical.
